  • 2027 CUHA Externship Request

    Veterinary students in their final or next to final clinical year of DVM or foreign equivalent degrees are permitted to visit and participate in clinical rotations with our veterinary students on a space available basis. In order to be considered for an externship, you will need to submit this form and the required attachments.
    • Please allow three to four months for processing and/or review of request
    • No more than four (4) weeks in one service
    • Six (6) week maximum visit

  • Upon successful approval of request, all visitors will be charged a $200 non-refundable application and processing fee to visit our facility. Visits will not be finalized until payment of the $200 non-refundable application and processing fee, along with required documents, have been received.
